Exercise and blood sugar

  1. As I've mentioned in previous posts, I have been a triathlete for several years and was diagnosed
    with type 1 several months ago. I am now training pretty much as I did before, but with much more
    attention to what I eat before, during, and after.

    I normally start my training sessions (be it swim, bike, or run) with a blood sugar somewhere
    around 100 and always more than 3 hours after my last injection of insulin (fast acting novolog).
    At the very last minute before I start, I will eat some carbs, but never more than 25 - 30 grams.
    In most cases, I end up needing more carbs after ~30 minutes in order to avoid hypoglycemia.
    Swimming seems to be the exercise where I am most prone to experiencing hypoglycemia. In my several
    months of training since being diagnosed, I have made some observations. Some are making sense to
    me and some aren't:

    (1) I am much less prone to hypoglycemia during high-intensity exercise (near anaerobic levels) than
    during low or moderate intensity exercise. As a good example, I have noticed that if I swim a
    bunch of hard 100 and 50 yard repeats, I can avoid hypoglycemia much longer. I did this for
    nearly an hour after eating 20g carbs last night and finished with a blood sugar of 85.
    Normally, if I do low-intensity continuous swimming for 30 minutes, I'll end up with a blood
    sugar in the 60's or 70's. I believe this is due to the secretion of counter-regulatory hormones
    during high-intensity exercise, but not during low-intensity exercise. Is this right?

    (2) After a training session, particularly a longer one, I do not need as much insulin as I
    normally would for a given amount of carbs. What I have not figured out is, does exercise
    multiply the effectiveness of insulin, or add to it? It seems like a little bit of insulin goes
    a long way after exercise.

    Attached is my standard type 1 and exercise post. Note the two books recommended at the end. I would
    suggest reading one or both. You don't state your age, but even if you are in your teens given your
    level of activity, progression to the point of 0 insulin production may be delayed. (Type 1s
    typically are diagnosed when 80% of their beta cells are non functional.) Be prepared to see changes
    in your responses that will require modifications in your bg management tactics.

    You can't really get out of basal insulin requirements but when exercise becomes part of the
    equation things become more interesting.

    While skeletal muscles are being exercised glucose can be utilised by them without the need for
    extra insulin. After you finish up your exercise session your glycogen levels will have been
    depleted somewhat. You get about an hour to replenish the glycogen reserves back to their prior
    levels without the need for extra insulin to cover the carbs. After the hour is up insulin will be
    required with food although residual insulin sensitivity should persist for at least the next
    several hours and up to several days, during which time insulin requirements are reduced.

    I think this should agree with some of your own experiences. Mileage always varies.
