CVD and insulin resistance

  1. Do any of you guys that see patients recognize that insulin resistance and cardiovascular problems
    usually go hand in hand? do you ever order GTTs for you patients to see how their insulin responds?

    I've been following all the research for syndrome X over the years, and it still seems like most
    doctors are out of date in terms of research and treatments. I went into my doctor years ago
    complaining about how hard it was to lose weight -- he just measured my TSH, which came back normal,
    and that was that.. only years later did I realize that I'm slightly insulin resistant -- I knocked
    carbs out of my diet, and 40 lbs just melted off. My cholesterol is finally optimal, although my BP
    is still a tad high (130-140/80-90).

    I've also read that many common ailments are related to insulin resistance -- two of these are male
    pattern baldness (I read a paper in 2000 that theorized this), and PCOS (one hypothesis is that PCOS
    and MPB are essentually the same). Does anyone have any observations that would support this?

    I think it's a really cool area to study, and I'm always looking at research papers on PubMed.

    Absolutely, on both accounts. You might look into "prediabetes" in your studies as well. Stay on
    track with this one!

    Low iron status and enhanced insulin sensitivity in lacto-ovo vegetarians.

    Hua NW, Stoohs RA, Facchini FS Department of Medicine, Division of Nephrology, San Francisco General
    Hospital, San Francisco, CA, USA.

    The efficacy of insulin in stimulating whole-body glucose disposal (insulin sensitivity) was
    quantified using direct methodology in thirty lacto-ovo vegetarians and in thirty meat-eaters. All
    subjects were adult, lean (BMI <23 kg/m2), healthy and glucose tolerant. Lacto-ovo vegetarians were
    more insulin sensitive than meat-eaters, with a steady-state plasma glucose (mmol/l) of 4.1 (95 %
    CI 3.5, 5.0) v. 6.9 (95 % CI 5.2, 7.5; respectively. In addition, lacto-ovo vegetarians had lower
    body Fe stores, as indicated by a serum ferritin concentration (microg/l) of 35 (95 % CI 21, 49)
    compared with 72 (95 % CI 45, 100) for meat-eaters To test whether or not Fe status might modulate
    insulin sensitivity, body Fe was lowered by phlebotomy in six male meat-eaters to levels similar to
    that seen in vegetarians, with a resultant approximately 40 % enhancement of insulin-mediated
    glucose disposal Our results demonstrate that lacto-ovo vegetarians are more insulin sensitive and
    have lower Fe stores than meat-eaters. In addition, it seems that reduced insulin sensitivity in
    meat-eaters is amenable to improvement by reducing body Fe. The latter finding is in agreement with
    results from animal studies where, no matter how induced, Fe depletion consistently enhanced
    glucose disposal.

    I might comment that insulin levels are poorly standardized, but that the fasting insulin level is
    the most commonly performed. Let me know if you find and studies showing GTT challenge and insulin
    level; I would be interested in seeing them.
