Clamp on index shifter

Six speed Shimano SIS or friction? We have both as will any LBS which commonly services bicycles.

I have a pair which I was about to junk, but they're braze-on. AFAIK the clamps are available
separately. Let me know if you want them (hrothgar19 AT yahoo DOT com)
