Campy Threaded Headset Help Needed

  1. Hi Folks -

    I recently received a new frame and fork with headset installed.
    I stipulated a Campy Chorus (threaded) headset, not their usual "what
    they have laying around".
    It's a canti brake model, so they also included a nice looking cable
    hanger. Two questions.

    1.) I noticed the hanger has a habit of turning out of relationship
    with the fork. It has a key that is small and doesn't seem to stay in
    the keyway of the fork. It also has one washer with key installed
    above and next to the adjusting cone. I'm wondering if there should
    be another washer with key above the cable hanger or ??? to prevent
    the hanger from turning out of sync.

    2.) I also noticed there is a space between the upper cup and the
    adjusting cone. In all the photos and illustrations of assembled
    headsets I find a space much smaller than that on my bike. In fact I
    can look into this opening and almost see the bearings. The upper
    adjusting cone's outer lip does not extend below the cups rim. If I
    remove the bearings, this slides easily together. Are the upper and
    lower bearings/retainers different on this model?
