Before you start taking it apart, make sure your derailleur is adjusted
properly. If you high limit is set too low, the shifter may not have enough
cable to index the high gear.

Try flushing it with the 'STI repair kit', WD-40 flush followed by a
spray lube inside whilst working the mechanism. No fix? Replace only.
Any STI will work, none last any longer, all are destined to fail
eventually. Campagnolo shifters are repairable, BTW..no more lost and
confused levers. Just need a competent LBS or us or Andy Muzi...

You could also put on a downtube shifter, then any brake lever, if your
frame as a downtube cable stop. These never fail.
